Mental health care has two problems at once: millions of people cannot find a therapist, and millions more see one who never quite gets them. BetterMind was built to solve both, matching people to therapists who have lived close to their lives, not just trained for their diagnoses.

Why we built this

Referrals have always been broken.

The standard process for finding a therapist is still what it has been for decades: a list from an insurer, a Psychology Today directory, or a tip from a doctor with fifteen minutes and no visibility into your actual life. You call. Nobody picks up. You try again. You settle for whoever had availability.

The match has always been the afterthought. We made it the product.

Before a therapist sees a single client through this practice, they answer the same questions you do. Not to screen them, but to give the matching engine something real to work with. When your answers and theirs align across the things that actually drive therapeutic fit, that alignment is the match.

We think people deserve to know what a system does with their answers before they give them. This page is that explanation. Not a marketing statement — a description of the machinery.

The boundaries

What AI does here. What it does not.

AI scores the match. Humans make the decision.

The matching engine produces a score from 0 to 100 for every therapist in the practice against your answers. That score is a signal — ranked ordering, a starting point. The therapist decides whether to accept the referral. You decide whether to proceed. AI does not make that call.

AI does not read your responses. It counts overlaps.

There is no language model reading what you wrote and forming a judgment. The scoring engine is arithmetic: for each question both parties answered, it computes a signed similarity score, weights it by the question's clinical relevance, and sums across everything. The formula is deterministic and produces the same result every time.

Your answers are not used to train any model.

Nothing you write in the questionnaire is used to improve or update any AI system. It is compared against therapist profiles and then stored only to produce your match. You can delete it any time.

The score is auditable.

Every match score is built from visible components — question by question, weight by weight. Nothing is a black box. If you want to understand why a therapist ranked where they did, that breakdown is available to you.

Under the hood

A formula, not a feeling.

Every scored question gets a value between −1 and +1. For a yes/no question, it is +1 if the answers agree and −1 if they do not. For a scale question, the distance between the two answers determines the score: no distance is +1, maximum distance is −1, and everything else is proportional.

The score is not a prediction of outcome. It is a measure of overlap — the best available proxy we have for fit before the first session happens.

Each scored value is multiplied by the question's weight, which was set by clinical judgment to reflect the expected relevance of that dimension to therapeutic fit. The weights have not yet been validated against outcome data; that study is on the roadmap. The weighted scores sum to a raw number. That number is scaled to 0–100, where 50 means as much agreement as disagreement and 80+ means strong measured alignment.

Questions where a therapist has no answer, or where you expressed no preference, are dropped from both sides rather than counted as a mismatch. You are only scored on what was actually answered.

Hard limits before scoring

Before a score is computed, four gates must pass: the therapist holds an active license in your state, accepts your insurance or self-pay, covers at least one of your presenting issues, and has not indicated they decline the kind of case you described. A therapist who fails any gate is not ranked — they are removed entirely.

Things the score does not mean.

It is not a clinical assessment

The questionnaire is a matching instrument, not a diagnostic tool. It measures the dimensions associated with alliance in the clinical literature. It does not assess severity, symptom profiles, or treatment appropriateness.

It is not a guarantee

A score of 90 means high measured overlap. It does not mean the relationship will work. Therapeutic fit is partly measurable and partly not. The score narrows the field; the work confirms it.

It is not permanent

You can retake the questionnaire, change your answers, or request a different match at any time. The score is a snapshot of your answers against the current therapist pool, not a permanent assignment.

The methodology

How the match
is actually built.

Matching is not a filter. It is a measurement. This page describes what goes into it, how the algorithm works, and what the score does and does not mean.

Step one

Both sides answer the same questions.

Before a client is matched, they complete a questionnaire. Before a therapist joins the practice, they complete the same one. The match is built from what both sides said.

Nine domains, 67 client questions

The questionnaire covers nine dimensions: who you are now, how you were raised, your life right now, what you have carried, how you are wired, faith and meaning, what you want from sessions, the therapist you picture, and logistics. Every dimension has a corresponding section for therapists.

Three tiers — you choose how deep

The 5-minute version covers 27 questions and 39% of the scored model, by weight. The 10-minute version adds 24 more, reaching 73%. The 15-minute version asks all 67 and fills in 100%. You can stop anywhere — skipped questions are left out rather than counted against you.

Private by design

Client answers are never shown to a therapist. Therapist answers are used by the engine and surfaced on profiles at a summary level — not question by question. Nothing is sold or used for advertising.

Step two

Four gates before scoring begins.

The scoring engine only runs against therapists who pass all four hard filters. A therapist who fails any gate is removed from the candidate pool entirely — they are not ranked low, they are not shown.

In-state licensure
The therapist holds an active license in the state where you want to be seen. This is a legal requirement, not a preference.

Insurance or self-pay
The therapist accepts your carrier. Self-pay always passes this gate. Medi-Cal and Medicaid are treated as interchangeable.
3
Specialty coverage
The therapist covers at least one of your presenting issues. A therapist with no specialty overlap on what you said brings you here is excluded.
4
No explicit decline
The therapist has not indicated they decline the category of case you represent. If you said you have active substance use and the therapist has said they do not treat active substance use, they are removed.

Why hard gates, not soft penalties?

A therapist who cannot legally see you in your state is not a 0/100 match — they are not a match at all. Treating a legal impossibility as a very low score would let them appear in results, which misleads you. Gates remove the candidates who cannot work before the ones who can are compared against each other.

The same logic applies to insurance: a therapist you cannot afford is not a low match. They are outside the viable set.

Step three

The scoring formula.

For every therapist who passed the gates, the engine scores the match question by question, weights by clinical relevance, and produces a 0–100 score.

Question scoring by type

Exact match (select questions)
+1 if the answers are identical, −1 if they differ. Used for categorical questions where a mismatch is meaningful regardless of direction — relationship status, living situation, substance use.
Ordinal distance (scale questions)
Score = 1 − 2 × |client − therapist| ÷ 4. On a 0–4 scale, identical answers score +1. A two-step gap scores 0. A four-step gap scores −1. Used for personality and style questions: directness, structure, pace of challenge, trust timeline.
Overlap (multi-select)
Jaccard-like: score = 2 × (shared items) ÷ (total unique items) − 1. A perfect overlap of identical lists scores +1. Disjoint lists score −1. Used for losses, life experiences, and things that would make the client quit therapy.
Coverage (must-have lists)
Score = 2 × (therapist hits on client's list) ÷ (client list length) − 1. Used for presenting issues and language: the question is whether the therapist covers what the client needs, not whether the lists match identically.
Attribute preference
Checks whether the therapist's demographic attribute matches what the client said they wanted. +1 for a match, −1 for a mismatch. Questions with "no preference" selected are dropped rather than scored. Covers gender, age range, race/ethnicity, faith, parenting status, recovery experience.

Weights and the final score

Each question carries a weight from 2 to 6, set by clinical judgment to reflect the expected relevance of each dimension to therapeutic fit. The weights have not yet been validated against outcome data. Higher weight questions have larger influence on the final score.

50 is the midpoint: as much agreement as disagreement across the answered questions. Above 50 means more agreement than divergence; below 50, the reverse. In simulation across 90,000 random client–therapist pairs, 86 % score under 50 and the median is 42 — so the midpoint is a high bar to clear for two people who have not chosen each other. Skipped questions are excluded from both raw and max — they do not hurt you.

Measured in simulation on 300 synthetic clients, each ranked against all 10,000 therapists: at the 5-minute tier, 37 % of clients have a best match in Strong; at the 10- and 15-minute tiers, the ceiling is lower (max scores 75 and 77) because more answers narrow the score toward the true overlap. The bands are reference points, not targets — a Good match is a genuinely useful starting point. State and insurance filters are part of Browse, not the questionnaire; they are not reflected in the band frequencies above.

Hard-fit penalties

Therapists can indicate client profiles they find hardest to work with. When a client profile matches a therapist's stated difficulty, 5 points are subtracted from the published score — regardless of how many questions were answered. This is not a gate: the therapist still appears in the ranked list. The penalty reflects a real clinical friction point the therapist themselves named.

What the score does not claim.

It is not a clinical prognosis

The questionnaire measures dimensions associated with therapeutic alliance in the literature. The weights are set by clinical judgment and have not yet been validated against outcomes. It is not a predictor of symptom reduction, treatment duration, or clinical outcome. No matching system can be that.

It is not permanent

Retake the questionnaire any time. Request a different match any time. No explanation is required. The score reflects your answers at a point in time — it is not a fixed assignment.

It is a starting point, not a conclusion

A score of 92 and a score of 78 are both strong starting points. The first session will tell you more than the number does. The score narrows the field to the therapists most likely to fit. The work confirms it.
